The Space Shuttle That Fell To Earth - BBC iPlayer

A look at the events leading up to & following the loss of Columbia in 2003. Interviews with NASA Managers, Engineers and families of the crew, some of which are quite hard to watch. Just as with Challenger they were warned, they buried their heads in the sand and a Crew died.

Whether this crew could have been saved is actually debatable but there had been similar incidents with foam coming loose from the external fuel tank and hitting parts of the Orbiter and just 2 missions before this one, a large chunk hit one of Atlantis' solid rocket boosters and dented it....yet senior NASA Management refused to believe a similar strike could cause fatal damage to a Shuttle Wing. Until it did..

Definitely worth a watch IMHO.

You would've thought that the brains at NASA could've worked out that despite it
being just light weight foam, it had the required amount of density/mass, coupled with the velocity that it was travelling to punch a hole in a weak spot in a wing.
