It could actually be dementia (or something along those lines), he can't get names right of people he's with and places he's at, can't pronounce the names of countries (some not remotely complicated) and slurs simple words. The covfefe thing was funny, as that was a typo, but some of the other stuff is a worry.

Makes you wonder what actual diagnosis is hidden in his health report that the US public and the wider world don't know about.
